Home Health Process
Step 1 – The journey begins with a physician’s order. Patients, their family members, or friends can request a home health order from their physician. Discharge planners or social workers at hospitals and nursing and rehabilitation centers may also reach out to the patient’s physician for this purpose. Additionally, staff from independent or assisted living communities’ wellness programs can initiate the process on behalf of a patient.
Step 2 – Upon receiving the order, Advent conducts a thorough check of health records and insurance benefits. Subsequently, we assign staff and arrange the initial visit.
Step 3 – Care commences. During the first visit, a skilled professional conducts a comprehensive assessment of the patient. A detailed care plan is then established in collaboration with the physician, the patient, their family, or any associated caregivers. Scheduled care continues to meet the patient’s needs.
Step 4 – Throughout the process, any changes, updates, or other important information are effectively communicated to the patient, their family, associated caregivers, and the physician.

Freedom of Choice
According to federal law, patients have the right to choose the institution, agency, or individual that provides their home health care. When home health care is recommended, patients should request their preferred provider.
Section 1802 of the Social Security Act aims to ensure that all Medicare patients have the freedom to choose their provider. It states: “Any individual entitled to insurance benefits under this title [i.e., Medicare] may obtain health services from any institution, agency, or person qualified to participate under this title if such institution, agency, or person undertakes to provide him such services.” This statement affirms patients’ freedom to select their provider of post-hospital services.
